Prognostic Significance of Epstein - Barr Virus in Nodal Peripheral T-cell Lymphoma, Unspecified: A Groupe d’Etude des Lymphomes de l’Adulte (GELA) Study Running title: EBV IN PTCL, UNSPECIFIED Scientific section: NEOPLASIA

چکیده
Peripheral T-cell lymphomas (PTCLs) are rare and have a dismal prognosis. The most frequent subtype is PTCL, unspecified. Epstein – Barr virus (EBV) has been detected in around 40% of cases, but its prognostic significance is not fully established. Lymph node samples from 110 patients with PTCL, unspecified included in LNH87 and LNH93 trials were available. EBV status was studied by EBER In Situ Hybridization (EBER-ISH). EBER-ISH showed positive cells in 45/110 patients (41%). Pretreatment characteristics were comparable between positive and negative cases, except for male gender (80% versus 60% respectively, p=0.02). Only 50% of patients achieved complete remission with a 5 year eventfree survival (EFS) and overall survival (OS) of 21% and 30%. EBER-ISH positivity was the sole factor linked with worse EFS with a 5 year probability of 11% for positive patients. In univariate analysis, factors affecting OS were EBER-ISH positivity, high LDH level and age >60 years. In multivariate analysis, EBER-ISH was associated with a worse OS in the elderly population. Time-dependent analysis showed that the negative impact of EBV was essentially seen in the first 2 years following diagnosis. These results warrant further studies regarding pathogeny and specific treatment approaches for EBV-associated PTCL patients. متن کاملImmunophenotyping of Nodal Peripheral T-cell Lymphomas and its Association with Epstein-Barr Virus
Background: Immunophenotyping in the rare group of nodal Peripheral T-cell Lymphomas (PTCL) exposes interesting features such as T-cell marker downregulation and paradoxically, the presence of reactive, clustered large-sized CD20 positive B-cells (B-cell proliferation). Epstein-Barr virus (EBV) has been suggested as a putative etiology in pathogenesis of B-cell lymphoma. We aimed to review the ...
متن کاملPrognostic significance of T-cell phenotype in aggressive non-Hodgkin's lymphomas. Groupe d'Etudes des Lymphomes de l'Adulte (GELA).
Peripheral T-cell lymphomas (PTCL) have been generally reported to have a worse prognosis than B-cell lymphomas (BCL). Because of their heterogeneity and scarcity, the outcomes of the different histological subtypes have not been compared. From October 1987 to March 1993, 1,883 patients with diffuse aggressive non-Hodgkin's lymphomas (NHL) included in the LNH87 protocol could be assessed for bo...
متن کاملPretreatment Epstein-Barr virus DNA in whole blood is a prognostic marker in peripheral T-cell lymphoma
Because there are few studies regarding the clinical impact of circulating EBV-DNA in peripheral T-cell lymphomas (PTCLs), we tried to evaluate the role of EBV-DNA in whole blood as a prognostic factor for PTCL. We retrospectively reviewed 110 PTCL patients with median age of 63 (20-94) years. Forty-seven patients (42.7%) showed positive results for EBV-DNA, and these patients also had stage II...
متن کاملThe Incidence of EBV in Diffuse Large B-Cell Lymphoma: A Comparative Study of Immunohistochemical and PCR Techniques
Background and Objectives: Epstein Barr Virus (EBV) is one of the members of herpesviridae family and a sub-category of Gamma herpes virinae. EBV, which normally has CR2 or CD21 receptors on B-lymphocytes, has mutagenic features for them. The virus plays an important role in causing some malignant cancers. About 30% of the cases with non-Hodgkin’s lymphoma are diffuse large B-cell l...
